16.6V on PPBUS is a problem. Shorted Q7030 I bet. Replace U7000, Q7030/35 and check resistance between pins 17/18, 27/28.

Define "short". This line will measure low just after removing power. Do a resistance measurement and make sure the board is fully discharged.

Something must be getting warm at 3A and 5V. You are putting 15W into the board! Use the good old lips and go over the board.

I cannot help you any further, that 15W must be going somewhere. Use thicker wires to the board and measure if the 5V is actually getting to the board and not already dropping somewhere in the PSU and wiring.
