Q7221 pin 5 = 0V

Pin 5 on Q7221 is low and that is correct. 3v3_s5 should be enabled. Seems like U7200 issue, check VREG3 and VREG5 and EN0. Replace chip.

Check again, 12.59V is way to high, should be 5V... If EN0 is high VREG3 should come high too, this seems to be a bad U7200...

Are you measuring the right pin? VREG5 should be 5v. You're getting 12.59v because you're measuring on C7272 instead of c7273 aren't you?

Check voltage on pin C7273!
